PMH Cares is for members of the Port Macquarie-Hastings community who are wanting:
You'll be asked to provide information relating to:
- Your name and contact details
- Participation as a recipient of support or provider of support
- Option to nominate to be a Neighbourhood Champion
- Current and valid Police Check
- Current and valid Working with Children Check
- Consent to Privacy conditions and Council's Code of Conduct2MB pdf(PDF, 2MB)
Once your registration has been submitted and reviewed, we'll connect you with your neighbourhood and/or Neighbourhood Champion who will get in contact with you to arrange the next steps.
Each group is self-managed. This means that your neighbourhood network for your street or rural community will decide how you work and what sort of help and contact you have with each other. We'll also be developing and providing a range of tools and resources to assist neighbourhood networks and Neighbourhood Champions and will provide this support ongoing.
Your contact details will be kept confidential apart from being used in ways that you consent to when you complete the form.
Risk and Responsibility
This is a local network of people who are helping each other. Everyone is responsible for themselves and for maintaining safe infection control practices. Each person participates in this network at their own risk.
Health advice is not our thing
