Required when a permanent food premises or business is in the sale/storage or preparation of food and drink.
This form is to be used if you are planning on selling food to the public from a temporary food stall, (e.g. at a market) to ensure that it is safe for human consumption and that the construction, fitting out and facilities for cleaning utensils, articles, hands, fittings and appliances in the stall are adequate.
